HOW DO I FIND A NETWORK DENTIST?
You have several ways to find up-to-date information on DeltaPremier® and DeltaPreferred Option® network dentists and to verify dentists’ network participation status:

• Search for a dentist via Delta Dental Plan of Illinois’ (DDPIL) website. Click the “Searching for a dentist?” box or click “Dentist Search” in the subscriber section of the website. The website is updated weekly and allows you to search for a dentist by city, state, ZIP code, specialty and/or name.

• Call DDPIL’s Automated Voice Response (AVR) dentist referral system, which is available 24 hours a day and provides names of DeltaPremier and DeltaPreferred Option network dentists over the phone or by fax. To access the AVR, call 1-800-452-1987.

• Call one of DDPIL’s Customer Service representatives at 1-800-452-1987. They are available to answer your questions and help you locate network dentists Monday through Friday from 8:30 a.m. to 5:00 p.m. (Central Time).

• Call your dentist’s office and ask if he or she participates with DeltaPreferred Option or DeltaPremier.
