About Your Group Dental Plan
May I use the dentist of my choice?
Yes. However, if you receive treatment from a DeltaPreferred Option (DPO) or a DeltaPremier network dentist, your out-of-pocket expenses may be lower. DPO dentists have agreed to accept reduced fees as payment-in-full. DeltaPremier dentists have agreed to accept Delta’s Maximum Plan Allowance (MPA) as payment in full, so you only have to pay your deductible and copayment. You do not have to pay any charges exceeding Delta’s MPA. More than 45,000 dentists in 61,300 dental offices throughout the country participate in the DPO network and more than 107,000 dentists in 135,500 dental offices throughout the country participate in DeltaPremier – nearly three out of every four dentists nationwide. How can I find out whether my dentist is a DeltaPreferred Option or DeltaPremier network dentist?
Since three out of four licensed dentists in the United States are DeltaPremier dentists and a large number are DPO dentists, there’s a good chance your dentist is a member of our network. To make sure, click here.
What if my dentist is not a DeltaPreferred Option or DeltaPremier network dentist?
Out-of-network dentists may “balance bill” their patients. This means if their fees exceed Delta’s Usual and Customary fees, they may bill their patients for the difference. You would have to pay that difference plus your deductible and copayment. You also might be required to pay the entire bill at the time of treatment. And you might have to complete and submit claim forms yourself.
Why not request that we invite your
dentist to join our DeltaPremier network? Send us your dentist’s name,
address and phone number via e-mail. Include your name and group number, and
let us know whether it’s OK to mention your name when we contact your
dentist. We’ll do the rest!
How do I file a claim?
DPO and DeltaPremier network dentists will complete and submit claim forms for you.
Delta accepts generic claim forms available from your dentist. However, if
an out-of-network dentist requests that you provide a claim form, you may obtain
a Delta Dental claim form from your benefits administrator, or you may download
a claim form here.

Call Delta’s Customer Service department at (800) 452-1987. We’re open on regular business days from 8:30 a.m. to 5 p.m. (Central Time). A Customer Service voice mailbox also is available 24 hours a day, with responses provided the next business day. For claim status, our Automated Voice Response system is available from 7:30 a.m. to midnight Monday through Saturday. Call the toll-free number above to access either of these services.
By the way, our Customer Service department receives the greatest number of calls on Monday and Tuesday. For faster service, you may wish to call on Wednesday, Thursday or Friday, when call volumes are lower.
